Exploratory association between vitamin D receptor gene variants and serum 25-hydroxyvitamin D levels in children with dengue hemorrhagic fever

Abstract

This exploratory analysis investigated the association between vitamin D receptor (VDR) gene variants and serum 25-hydroxyvitamin D [25(OH)D] levels in children with dengue hemorrhagic fever classified according to Vietnamese Ministry of Health guidelines in the Mekong Delta region of Vietnam. A prospective observational study was conducted on 267 children younger than 16 years old treated at Can Tho Children’s Hospital and Ca Mau Obstetrics and Pediatrics Hospital from February 2024 to December 2025. The median age was 10.9 years old; 43.8% were female; severe dengue hemorrhagic fever and shock accounted for 34.5% and 34.1%, respectively. Serum 25(OH)D was measured during the acute phase, and four VDR single-nucleotide polymorphisms, including rs2228570 (FokI T>C), rs7975232 (ApaI G>T), rs731236 (TaqI T>C), and rs1544410 (BsmI G>A), were genotyped. Multivariable linear regression was used to assess associations between VDR variants and serum 25(OH)D. The median 25(OH)D concentration was 28.8 ng/mL. After adjustment, rs7975232 (ApaI G>T) under the T-allele carrier model showed an exploratory association with lower 25(OH)D levels (β = -3.54; 95% CI: -6.53 to -0.54; p = 0.021). The G-T-T haplotype (BsmI–ApaI–TaqI) also showed an exploratory association with lower 25(OH)D levels (β = -3.22; 95% CI: -5.92 to -0.52; p = 0.020). These findings should be interpreted as exploratory signals and require confirmation in independent studies.
