Association Between Late Gadolinium Enhancement Quantification, T1 Native and Extracellular Volume by Cardiac Magnetic Resonance in Patients With Hypertrophic Cardiomyopathy

Abstract
This study aimed to compare different late gadolinium enhancement (LGE) quantification techniques on cardiac magnetic resonance (CMR) and to evaluate the relationship between LGE, native T1, and extracellular volume (ECV) in patients with hypertrophic cardiomyopathy (HCM). A cross-sectional descriptive study was conducted on 33 patients with HCM who underwent CMR at Bach Mai Hospital. LGE was quantified using the 2SD, 4SD, 5SD, 6SD, 10SD, and full-width at half maximum (FWHM) methods. LGE differed significantly among quantification techniques (p < 0.001). The 2SD method yielded the highest LGE values, whereas higher SD thresholds and FWHM produced substantially lower values. Among the evaluated techniques, the 5SD method demonstrated the strongest correlation with ECV (r = 0.430, p < 0.05), while 6SD showed the highest correlation with native T1 (r = 0,355, p>0,05). These findings suggest that intermediate threshold techniques, particularly 5SD, may better reflect myocardial fibrosis burden in patients with hypertrophic cardiomyopathy.
