Association between the ApoB/ApoA-I ratio and coronary artery lesions in patients with acute coronary syndrome

Abstract
The objective of this study was to investigate the association between serum ApoB, ApoA-I, and the ApoB/ApoA-I ratio and the severity of coronary artery lesions assessed by the Gensini score in patients with acute coronary syndrome. Employing an analytical cross-sectional design, the study enrolled 113 patients treated at two general hospitals in Dong Nai Province from July 2025 to April 2026. The results showed that ApoB levels and the ApoB/ApoA-I ratio were significantly higher in the moderate-to-severe coronary artery lesion group than in the mild lesion group (p = 0.019 and p = 0.023, respectively). ApoA-I levels did not differ significantly between the two groups (p = 0.093). Multivariable logistic regression showed that the ApoB/ApoA-I ratio remained significantly associated with moderate-to-severe coronary artery lesions after adjustment for covariates in the multivariable model (OR = 1.27, 95% CI: 1.01 – 1.62, p = 0.048). The ApoB/ApoA-I ratio may be a useful marker for assessing the severity of coronary artery lesions in patients with acute coronary syndrome.
