Results of management of blunt cardiac injury in Vietduc hospital

Abstract
Unlike cardiac injury, cardiac trauma can have a wide variety of clinical manifestations. The purpose of this study is to develop a protocol for the management of cardiac trauma based on clinical conditions suitable to practice conditions in Vietnam. The study was conducted with 34 patients, whose mean age was 40.5. The main cause of heart trauma is traffic accidents (20/34 cases). The clinical types according to the degree of cardiac compression are: fulminant: 3, acute: 10, subacute: 13, non - compressive: 8. 58.8% had chest trauma, 41.2% had chest trauma with breastbone fracture. All patients with fulminant form and most patients with acute form were operated with extensive sternotomy and the patients had good results at discharge. Most subacute forms (11/13 cases) were drained of Marfan and non - compressible patients were treated conservatively in 4/8 cases. There were two deaths: 1 due to infection in a patient with multiple severe trauma, 1 due to excessive damage to the heart chambers, leading to uncontrolled bleeding after surgery. The clinical form based on the acuteness of cardiac tamponade is an important basis for performing cardiac trauma surgery .
