12. Rate and predictive factors of futile recanalization after mechanical thrombectomy for anterior circulation: a single-center study

Abstract

Futile recanalization was defined as a successful recanalization achieved after mechanical thrombectomy (MT) without a good outcome at 3 months (modified Rankin Scale (mRS) ≥ 3). From December 2020 to February 2022, 80 patients with acute ischemic stroke due to large vessel occlusion within 4.5 hours were treated with MT with and without intravenous thrombolysis (IVT) at Bach Mai hospital. There were 50 males (62.5%) and 30 females (37.5%) with median age 65.5 (IQR, 59 - 74), median baseline NIHSS score 12 (IQR, 10-16), baseline ASPECTS score 7 (IQR, 7 - 8). Among them, 40 patients (50%) received IVT prior to MT. Successful recanalization was achieved in 72 patients (90%). Futile recanalization is relatively common (accounting for 33.3%). Older patients (> 70 years), high baseline NIHSS score (> 14 points), and higher number of passages in MT (> 2 times) were independent prognostic factors of futile recanalization.
