15. Trapezius myocutaneous flap for head, neck, back defects caused by cancer: application and results

Abstract

The head, neck, back are common occurrence sites for skin and soft tissue cancer, which usually cause large defects in need of reconstruction. Trapezius myocutaneous flaps have a constant blood supply, are easy to dissect, has large skin paddle, therefore they are ideal materials for head, neck, and back reconstruction. Objective: Describing the result of trapezius myocutaneous flap reconstruction in head, neck, and back cancer. Method: Serial case report with sixteen patients with histopathically diagnosed cancer in the region of the head, neck, and back, whose defects were reconstructed by using trapezius myocutaneous flap in Vietnam National K Hospital. Results: Largest defect of 22x16cm, smallest defect of 12x9cm. 75% of the defects were in or near the scalp area. 25% of the defects were in the upper back area. 87.5% of the flaps have uneventful postoperative courses, 6.25% of the flaps has partial necrosis and 6.25% of the flap has total necrosis. Conclusions: Trapezius myocutaneous flap is highly suitable for reconstruction following head, neck, back cancer excision.
