Early outcomes of laparoscopic cholecystectomy after percutaneous gallbladder drainage for severe acute cholecystitis at Hanoi Medical University Hospital

Abstract
A case series study involving 14 patients with grade III acute cholecystitis was conducted at Hanoi Medical University Hospital from 2020 to 2025. These patients were treated with emergency percutaneous transhepatic gallbladder drainage followed by laparoscopic cholecystectomy either during the same hospitalization or upon follow-up when their condition stabilized. The drainage procedure significantly improved clinical symptoms such as fever, abdominal pain, and infection markers, as well as organ dysfunction indicators. After drainage, all patients underwent laparoscopic cholecystectomy, with only one case requiring conversion to open surgery. No intraoperative complications or postoperative adverse events were recorded. The average hospital stay following surgery was 5 days. The results demonstrate that emergency gallbladder drainage is an effective and safe initial treatment option for patients with severe acute cholecystitis. It helps stabilize the patient’s general condition prior to laparoscopic cholecystectomy, ensuring the feasibility and safety of the procedure.
