Surgical treatment of epithelioid hemangioendothelioma in the liver: A case report

Abstract
Epithelioid hemangioendothelioma (EHE) of the liver is a rare vascular tumor that is often misdiagnosed as hepatocellular carcinoma or metastatic lesions. Histopathology plays a crucial role in establishing an accurate diagnosis. A 37‑year‑old male with no prior medical history, presenting with right upper quadrant pain and mild weight loss over several weeks. Clinical examination was unremarkable. Magnetic resonance imaging revealed multiple hepatic nodules: the largest measuring 27×12mm in the left lateral segment, with additional lesions in segment VI (8×12mm) and segment VIII (12×15mm). A preoperative liver biopsy was performed. Histological findings were consistent with hepatic EHE. After multidisciplinary consultation, the patient underwent surgical resection. The operation was successful, with complete removal of the tumors and no invasion of major vessels or adjacent organs. Postoperative pathology and immunohistochemistry confirmed the diagnosis. The patient recovered well, with no complication, and remains under regular follow‑up. This case contributes to the characterization of clinical and imaging features of HEHE and emphasizes the indispensable role of histopathology in achieving diagnostic accuracy for this rare disease.
