Tóm tắt

Nghiên cứu này nhằm đánh giá kết quả bước đầu phẫu thuật u màng não vùng lỗ chẩm và dốc nền bằng đường mổ xa bên. Đây là vùng giải phẫu phức tạp tại nền sọ, nơi tập trung các cấu trúc quan trọng như thân não, động mạch đốt sống và các dây thần kinh sọ thấp (IX-XII), khiến phẫu thuật qua đường mổ này gặp nhiều thách thức. Phương pháp: nghiên cứu mổ tả tiến cứu trên 05 bệnh nhân được phẫu thuật từ tháng 01/2023 đến 06/2025. Kết quả: 5 ca gồm 4 nữ, 1 nam; tuổi trung bình 58 (từ 48 đến 65). Kích thước u trung bình là 29,6mm. Tỉ lệ lấy toàn bộ u đạt 100%. Các biến chứng như rò dịch não tủy, liệt dây VI, chậm liền vết mổ đều điều trị bảo tồn thành công. Không có biến chứng nặng hoặc tử vong. Kết luận, đường mổ xa bên là đem lại một hành lang tiếp cận u màng não vùng lỗ chẩm và dốc nền ở phía trước-bên đồng thời hạn chế việc vén não, bảo tồn tốt các cấu trúc thần kinh - mạch máu quan trọng.
