The association of phosphorylated insulin-like growth factor binding protein-1 (phIGFBP-1) and cervical parameters in the prediction of preterm delivery

Abstract

This prospective cohort study was conducted on 150 pregnant women diagnosed with threatened preterm labor at Can Tho Obstetrics and Gynecology Hospital. Clinical findings showed a mean cervical length (CL) of 27.09 mm (with 36.67% having CL < 25 mm) and a phIGFBP-1 positivity rate of 48.7%. The preterm birth rate in the phIGFBP-1 (+) group was 54.8%, significantly higher than the negative group (24.7%) (p < 0.001), with a 3.7-fold increase in the risk of preterm birth. The combined predictive model-incorporating Uterocervical Angle (UCA), Cervical Length (CL), and phIGFBP-1-yielded an Area Under the Curve (AUC) of 0.731, compared to CL measurement alone (AUC = 0.696). This integrated model provides a risk stratification tool, enabling the identification of high-risk pregnancies for timely intervention while minimizing overtreatment and unnecessary hospital admissions for low-risk groups
