13. Outcomes of lower lip reconstruction after cancer resection using local flaps

Abstract

This study assesses the outcomes of local flap reconstruction for soft tissue defects of the lower lip following oncologic resection. A clinical interventional study was conducted on 24 patients treated at K Hospital from September 2017 to March 2023. Results: The rate of complete flap survival was significantly higher in patients with defects involving 1/4 to 3/4 of the lower lip length (100%) compared to those with defects exceeding 3/4 of the lip length (52.6%) (OR = ∞; p = 0.0039). Flaps harvested from the lower lip, adjacent mucosa, or submental region demonstrated a favorable trend over those based on the upper lip and cheek (OR = 3.88; p = 0.159). However, no statistically significant difference in functional outcomes was observed between the two groups (p = 1.0). Conclusion: Local flaps are suitable for reconstructing lower lip defects involving 1/4 to 3/4 of the lip length, providing reliable functional and aesthetic results. Flap selection should be tailored based on defect location, size, extent, and depth. Flaps from the lower lip, mucosa, or chin tend to offer superior aesthetic results compared to upper lip or cheek flaps, although functional outcomes are comparable.
