77. Strongyloides steroralis hyperinfection associated with e.Coli sepsis: A case report from Bach Mai Hospital

Abstract
In this article, we report on a case of concurrent hyperinfection strongyloidiasis and Escheria coli (E. coli) septic in a polymorbid patient, diagnosed and treated at the Institute of Tropical Diseases, Bach Mai Hospital. This is a severe and uncommon presentation due to its atypical clinical course. A 79-year-old male patient with a history of diabetes mellitus, hypertension, and immune thrombocytopenic purpura (IPT), receiving Medrol 16 mg/day was admitted for hemoptysis. The patient’s past medical history included multiple episodes of hemoptysis and melena but initial symptoms did not include abdominal pain or diarrhea. Subsequently, the patient developed fever, diarrhea, vomiting post admission. Bronchoscopy and stool test results were positive for strongyloides larvae. Serological testing for strongyloidiasis results was positive. Multiple blood cultures were positive for E. coli. He was successfully treated with Ivermectin, specific antibiotic and nutritional support. Two months following discharge, the patient’s clinical condition improved significantly, with no recurrence of hemoptysis. Severe strongyloidiasis co-infected with septimia is a severe, complex, rare case with a high mortality rate. Further research is needed on this particular case.
