Factors associated with the timing of emergency endoscopic retrograde cholangiopancreatography in patients with gallstone-induced acute pancreatitis complicated by acute cholangitis

Abstract

A study was conducted on 86 patients with gallstone-induced acute pancreatitis complicated by acute cholangitis who underwent endoscopic retrograde cholangiopancreatography (ERCP) at Can Tho Central General Hospital from June 2024 to September 2025. The mean age of the patients was 67.6 ± 15.4 years with a female predominance of 66.3%. Upon admission, 100% of patients presented with abdominal pain, and jaundice accounted for 57.7%. According to the 2012 Atlanta classification, severe acute pancreatitis accounted for 26.7%. The rate of emergency ERCP in the study was 52.3%. Multivariable logistic regression analysis identified two independent factors associated with the indication for emergency ERCP, including: Mean total serum bilirubin concentration at admission (p = 0.003; OR = 1.02) and severe acute cholangitis based on the Tokyo Guidelines 2018 at admission (p = 0.001; OR = 5.92).
